SA EMS FIELD PROTOCOLS:
IN REAL CONTEXT
The HPCSA Clinical Practice Guidelines translated into real road scenarios, acute decision trees, statutory pharmacology monographs, and cadre backup triggers.
Indications & dosages strictly aligned to HPCSA CDST v1.3 & Annexure A; contraindications & adverse effects referenced from SAMF (14th Ed) & emergency pharmacotherapy.
Pre-Hospital Shock Index, GCS, airway decision trees, and permissive hypotension matrices.
Push-Dose inotrope formulas, pediatric weight & dosage drivers, and Parkland burn calculations.
Immediate clinical escalation thresholds across BAA, ECA, AEA, ECT, CCA, NDip, and ECP.

Real Road Case Simulation
Scenario Walkthrough: Rural Polytrauma (TBI + Hemorrhagic Shock)
Case Presentation: 28-year-old male, unrestrained driver in a high-speed rollover on a regional gravel road (90-minute transport time to referral center). GCS 6 (unresponsive, snoring airway, clenching jaw), compound left femur fracture with arterial spurting, rigid/distended abdomen. Vitals: BP 78/42 (MAP 54), HR 138 (thready), RR 8/min (shallow), SpO2 84%.
Exclusive Previews: Built Into Every Case
Here is a preview of the 3 tactical tools embedded in Case 01: Drug Profiles, Assessment Tools, and Resuscitation Calculations.
Adrenaline (Epinephrine) Inotrope / Push-Dose(Sympathomimetic Catecholamine)
Pre-Hospital Shock Index & Permissive Hypotension Target
• SI > 1.0 (Case 01: 138 ÷ 78 = 1.76): Indicates massive occult internal bleeding. Mandates immediate push-dose vasopressor support & pre-alerting trauma surgical theater.
Adrenaline Infusion & Compounding Calculator
• Clinical Verification: Dynamic saline withdrawal vs direct injection rules, paediatric safe-start dosing (0.1 mcg/kg/min), and fluorescent barrel labeling.
